By signing below, I hereby grant consent for 24h Childcare to provide childcare services to my child,

Terms of Consent

 

1. Care Provision: I consent to 24h Childcare providing childcare services to my child, including but not limited to, basic care, companionship, feeding, bathing, and engaging in activities.

2. Caregiver Access: I grant permission for 24h Childcare caregivers to enter our home to provide care for my child.

3. Medical Care: In the event of a medical emergency, I authorize 24h Childcare to seek medical attention for my child, including calling emergency services or transporting my child to a medical facility.

4. Communication: I consent to 24h Childcare communicating with me regarding my child's care, including updates, concerns, and scheduling.

 

Acknowledgments

 

1. Home Safety: I acknowledge that it is my responsibility to ensure our home is safe and free from hazards for my child and the caregivers.

 

Additional Consents

 

2. Emergency Contact: I authorize 24h Childcare to contact anyone on the emergency contact list in case of an emergency if the parent is unreachable.
